The use of coal tar is declining as newer compounds effective against the different forms of psoriasis are replacing it. However, it still has the advantages of being low cost and causing less systemic toxicity as compared with more modern therapies.
Coal tar is still used to treat a variety of skin disorders, particularly conditions where the skin is flaky and scaly.

• Scalp psoriasis
• Palmoplantar pustulosis
• Plaque psoriasis
• Atopic dermatitis

Exactly how coal tar works to treat these conditions is not completely understood. It appears to have antimicrobial, antipruritic (reduce itching) and keratoplastic (normalise keratin growth in the skin and reduce scaling) effects.
How to use coal tar

• Apply it at bedtime if possible to avoid daytime inconvenience of the smell and staining
• Leave on skin for at least 2 hours unless otherwise directed by your doctor
• If treating the scalp, apply with a downward stroke in the same direction as hair growth
• Cover or wrap with bandages to help the preparation stay in place and prevent staining. Do not use plastic wrap as this may cause irritation and infection
• Do not apply to infected, blistered, raw or oozing areas of the skin
• After applying coal tar preparations, protect the treated area from direct sunlight and do not use a sunlamp unless otherwise directed by your doctor.

Undesirable Effects

Most patients tolerate coal tar preparations well. It may initially cause mild stinging or skin irritation as your body gets use to the medicine. However, if this continues stop using the preparation and see your doctor.
When used on the scalp, it may temporarily discolour bleached, tinted, light blond or grey hair. Coal tar also stains skin and clothing. The stain on the skin will wear off after you have stopped using the preparation.
Coal tar may cause photosensitivity, hence the need to stay out of direct sunlight when using these preparations. In some instances your doctor may use the Goeckerman regimen, which is a combination of coal tar with UVB phototherapy. This has been found to be particularly successful for treating psoriasis.
There is some controversy over the carcinogenic (cancer-causing) potential of coal tar. In animal studies, it has been shown to increase the chance of skin cancer. However, in human studies an increase in the incidence of cancers has not been found after 75 years of documented use.

انا طالب صيدله قدرت افهم منه حوالي 85% بس راح احط لكم المعنى بشكل عام
- قطران الفحم الحجري :- سائل سميك اسود حضر بواسطه مشتقات تصنيع الفحم والنفط
القطران حضر قديماً لعلاج مرض الصدفيه لانه يقلل الحكه والقشور والالتهابات .. آليه عمله غير معروفه الى الان .. القطران له بعض الاثار الجانبيه -مزعج للمستخدم و يصبغ ويلون الملابس- وايضا له رائحه قويه
وايضا يتوفر في مستحضرات من غير وصفه طبيه ( over the counter) مثل الشامبوهات والكريمات والزيوت والمراهم وايضا يتوفر بتركيزات تتطلب وصفه طبيه من مختص
والقطران متوفر وآمن وغير مكلف ويعتبر خيار جيد لملايين مرضى الصدفيه وبعض الامراض الجلديه
تركيز القطران من 0.1% - 5% امن ومؤثر وفعال للصدفيه وتشير الدراسات ان هذي النسب والتركيزات غير مسرطنه وان المواد اللتي يتكون منها القطران عددها حوالي 10000 ماده كيميائيه تقريباُ وحوالي 50% فقط تم تحديدها التعرف عليها ..

ويمكن الحصول على القطران في اي تركيز يطلبه الطبيب عن طريق تحضيره بواسطه صيدلاني
في ماذا يستخدم القطران؟؟؟
بدأ استخدام القطران بالتناقص بسبب اكتشافات اكثر حداثه لعلاج الصدفيه مما ادى الى استبداله بها ولكنه يفضل القطران عنها بسبب التكلفه المنخفضه والثاثيرات السميه التي تعتبر اقل من العلاجات الحديثه
ومازال يستخدم بكثره في علاج صدفيه الراس والصدفيه اللويحيه وبعض الامراض الجلديه
واليه عمل القطران غير معروفه ومفهومه بالضبط ويظن لخصائصه المضاده للميبكروبات تطبثه لنمو الكيراتين والحد من تكاثره .
كيف يستخدم القطران؟؟
عمل تركيبات مع المركبات الاخرى لصنع كريمات وصابونات ومراهم و لوشانات وجل ودهانات ومعاجين ومحاليل ويتم كتابه التركيبات والارشادات على اللاصقه الخاصه بمستحضرات القطران واللتي تعتمد على عده قواعد اساسيه وهذه بعض النصائح :-
وضعه ليلا اذا كان ممكناُ تجنبا للاتساخ والرائحه المزعجه
ان يترك على الجلد مده لاتقل عن الساعتين او حسب توجيهات الطبيب
اذا كنت تعالج الراس ضعه في اتجاه نمو الشعر وليس عكس اتجاهه
وضع ضماده من قماش تجنبا للاتساخ ولا يغى ببلاستيك لانه قد يسبب تهيج والتهاب
لايوضع على الجلد الملتهب او المصاب او المجروح
تجنيب المنطقه المعالجه بالقطران من اشعه الشمس المباشره
الاثار الغير مرغوب بها ؟؟؟
قد يحدث تهيج وحرقه متوسطه الى قويه عند اغلب المرضى عند استخدام القطران .. اذا استمر التهيج والحرقه يجب التوقف واستشاره ومراجعه الطبيب
عند استخدامه على فروه الراس فانه قد يصبغ الشعر مؤقتاُ الى الاشقر الخفيف او الرمادي ( يعني شيب ) وايضا قد يترك علامات وبقع على الملابس والجلد وسيزول هذا عند التوقف عن الاستخدام
عند استخدام القطران يحدث تحسس من اشعه الشمس لذا يجب البقار بعيداُ عنها وبعض الاحيان قد يطلب الطبيب المزج بين العلاج بالقطران مع العلاج بالاشعه الفوق بنفسجيه ووجد هذا العلاج فعال جداُ لعلاج الصدفيه
يوجد بعض الجدل حول اذا ماكان مسبباُ للسرطان ام لا توجد دراسات عملت على الحيوانات وجد انه قد يسبب سرطان الجلد ام على الانسان لم تسجل اي حاله مع موثقه يعد استخدام القطران اكثر من 75 عام

صدق بو ريماس انا سويت هذه الطريقة وموظف الاستقبال ما يدقق في الوصفة بصراحة انا اضفت coal tar solution 20% 2L وكان الخط واضح انه غير خط الدكتور ولا حتى لون القلم كان نفسه وقالي موظف الاستقبال كم تبي يعني لو قلت عشرين لتر عطاني الشغلة تجارية بحته ولكن ما ادري ليش مصرين على الوصفة وعلى فكرة سألت موظف الاستقبال ليش تصرون على الوصفة قال هيئة الغذواء والدواء راح تسألنا اخر العام وين صرفتوا تلك الكميات !! انا غير مقتنع بكلامه لأني ابن البلد واعرف ماحولك احد!! وياليت كلامه صحيح

نعم يقبلون وصفات من خارج المملكه
انا رسلت لاخوي وصفه من اليمن وجاب لي القطران وبدأت استعماله لي 3 ايام
القشور في تساقط الحمد لله
